Health Form

In accordance with NC General Assembly Law 2015-222 (formerly referred to as HB 13):

Effective 2016-2017, all students entering NC public schools for the first time must provide a Health Assessment Form upon school entry.  (If the form is not presented on the first day of attendance, the student will have 30 days in which to provide this form or be excluded from school.)  This applies to:
  • all students transferring from out-of-state and entering NC public schools for the first time
  • all students transferring from private schools or homeschools into NC public schools for the first time
  • all Kindergarten students (This replaces the old Kindergarten Assessment Form.)
The law differs slightly from immunization attendance requirements, in that students must not be penalized academically.  While excluded, the student must be provided with materials in order to make up all missed work.
